1. Please fill out the form online and when you have completed it.

2. Print the web page out on your printer.

3. Fill out the remaining top information with ink pen.

4. Please be sure all important areas are fill out and the total is correct at bottom of form.

5. If you are paying by credit card.  You can fax the form to: (949) 218-4357.

6. Or have your student return the form with the credit card information
     and / or check to their teacher or the office.
